Treating Plantar Fasciitis

Plantar Fasciitis (PF) is a degenerative syndrome of the plantar fascia resulting from repeated trauma at its origin at the calcaneus1.

It is reported to be the most common cause of inferior heel pain in adults2.

Plantar fasciitis is also known as painful heel syndrome, heel spur syndrome3, runner’s heel, policemen’s heel, subcalcaneal pain, calcaneodynia and calcancal periostitis.

Treating the causative factors of plantar fasciitis is key to designing a treatment for your patient.Using the point of pain test will indicate if the pain is Lateral, Central or Medial.Valmassey refers to plantar fasciitis as affecting both pes planus and pes cavus foot types4 with opinion being that different pain regions may be suffered depending upon the foot structure (watch video).Generally speaking the following often applies:

Lateral Heel Pain is often associated with an uncorrected forefoot valgus and pes cavus foot type.

Central Heel Pain often indicates a forefoot valgus is present, in addition to rearfoot pronation.

• Medial Heel Pain generally indicates the patient will exhibit excessive rearfoot pronation.

In each case, treatment will involve prescribing an orthotic device to control rearfoot pronation and provide biomechanical correction. In the cases of Central and Medial pain, a heel deflection (referred to as a ‘horseshoe deflection’) in the orthotic can also assist by relieving pressure on the attachment. Low Dye Strapping can be used to provide short term pain relief, as it mimics the support and control offered by an orthotic device.
Shockwave therapy has also been used to some effect in breaking up scar tissue. Anti-inflammatory medications, massage and surgery are generally less successful in the long term treatment of Plantar Fasciitis.

  1. Why are certain products now restricted, I’ve bought them for several years and was trained in their use as part of my training.
  2. The MHRA rules have not changed. For many years practitioners who have undergone training have been allowed to dispense certain products that are predominantly foot health related, their role was similar to that of a qualified pharmacist. With a vast increase in online supplies of pharmaceuticals MHRA guidelines are now much stricter when it comes to checking the actual purchaser of the products.

 

  1. I’m HCPC Registered – Can I buy as before?
  2. You can buy the same items as previously, but we do need a copy of your HCPC registration number on file, this can then be checked independently by MHRA should their inspector request it.

 

  1. I’m a Foot Health Practitioner Trained With Certain Restricted Products – How do I prove my qualifications?
  2. If you are registered on the Foot Health Practitioner Register, then we require your registration number similar to HCPC above. For non registered practitioners we require a scanned copy of your qualification certificate from your training school.

 

  1.  Does this now allow me to buy POM medicines/Anaesthetic?
  2. No – The only practitioners allowed to purchase Anaesthetic for Nail Surgery use are those who have the correct certificates of additional training in prescription only medicines or  anaesthetic use and nail matrixectomy.

Speech-BubbleDLT Chiropody & Podiatry: Thanks for joining us I have a few questions the first is: Your nail polishes are free from formaldehyde, formaldehyde resin, toluene, parabens, and phthalates, what does these bad chemicals do to the nail that are in traditional nail polishes?

Speech-Bubble Rachel Bacheler: While we don’t know truly how porous the nail bed is, many podiatrists believe that formaldehyde and other harsh chemicals (traditionally added to nail polishes) not only cause nails to be dry and weak, but are potential health hazards. Most people believe it’s best to avoid them. There are many documented reports about the long-term exposure to your above mentioned toxins.

Speech-BubbleDLT Chiropody & Podiatry: How does Dr.’s Remedy compare to other good quality nail varnishes?
How long lasting are they and how do you get the best from the products?

Speech-BubbleRachel Bacheler: It’s difficult to compare Dr.’s REMEDY to other polishes that are not doctor formulated and enriched. Speech-BubbleRachel Bacheler: Not sure I understand the question – are you asking if patients can use another brand’s base coat? If so, it’s defeating the purpose.

Speech-BubbleDLT Chiropody & Podiatry: No I mean can a patient use your brand of base coat then use say a cheaper brand of colour on top of that base coat, and then will that affect the fungal nail?

Speech-BubbleRachel Bacheler: It is probably possible to do so, although we have never tested how other brands perform in conjunction with Dr.’s REMEDY. We do know that we have quality controlled the way our base, color and top coat function and our strongest recommendation is to apply ‘as recommended’.

Speech-BubbleDLT Chiropody & Podiatry: So will the formaldehyde, formaldehyde resin, toluene, parabens, and phthalates of the cheap brand get get through the nail?

Speech-BubbleCaroline Christer: Hello Rachel, my husband and I run a multi-disciplinary practice that includes podiatry, beauty and a range of other treatments. I am interested in what the additives are in your product that make it different / homeopathic? What are the long term effects of these additives?

Speech-BubbleRachel Bacheler: Thank you for your question, Caroline. First I will answer from DLT: again, we haven’t tested how other brands effect our base or top coat. It is possible that the toxins you mention could still yellow the nail. Our base coat is only the first step.

Hi Caroline! The additives in our polish are tea tree, garlic bulb extract, lavender (all natural anti-fungal/ anti-bacterials), also added: vitamins c&e and wheat protein. We are also adding biotin to some of our products starting Spring 2015.

Speech-BubbleCaroline Christer: Have you undertaken clinical trials on these products Rachel? I am interested to know the effects of the essential oils on a fungal nail, given that this is an extremely difficult infection to eradicate.

Speech-BubbleRachel Bacheler: Our shaded nail polish can be applied directly to the nail. We have older clients as well that prefer just one step. Basic Base coat aides with durability and eliminating staining. Furthermore, Calming is a nice final sealant and a great final coat for ‘nail biters’!
Caroline, as mentioned previously, Dr.’s REMEDY is NOT a medicine. While we are researching a clinical trial at this time, we need patients to understand that a truly fungal nail condition will require medication.

Speech-BubbleSarah Sadler: I love all the colours that you do at the moment and find that there is no real age limit to individual colours that you can find with other nail colour companies. I also like the fact that your reds and blues don’t stain the nails which can happen even with base coat applied.
Do you do a French polish set???

Speech-BubbleRachel Bacheler: Our unisex line launched this past Fall. The matte top coat is really what makes any neutral shade less obvious (for a man). If a man needs to hide discoloration on his nails, for example, he would chose a shade such as Loyal Linen and use MODEST Matte top coat to take away the shine. We have fantastic testimonials from men, here in the states that are loving this concept. (and remember, Modest Matte is really cool for women as well – it makes dark shades look ‘brushed’)

Speech-BubbleRachel Bacheler: Sarah, sure! Again not sure how another brand’s polish performs with our base coat, but you can test it out. Note that our brand isn’t about the trends per se, but rather the health of the nail.

Nails with a yellowish discolouration

yellow discoulored nailsFinger and toe nails are regarded as the reflection of the physical state of health. If they show a yellowish discolouration this is usually due to a cosmetic cause. In many cases, however, there is also a medical problem, such as a metabolic disorder.

Patients are quickly suspected of not looking after their finger nails when they are discoloured. Smoking often embeds toxins in the nails. They attack the callus plates and lead to colour changes.

Wrong care, trauma or constant strain due to excessively tight shoes are also regarded as causes of yellowish finger and toe nails. Physical influences such as colour forming bacteria or x-rays may also be causes. Yellow nails often occur after pedicures. The cause is deemed to be the solvents which are contained in many – especially coloured – nail polishes.

From yellow to crumbly
Colour changes and excessively soft or brittle nails, crosswise grooves (ridging), a detaching nail plate or crumbly nails may not always be the patients fault but due to a physical disorder.  In yellow nail syndrome, a growth disorder thickens the nails and gives them a yellowish discolouration (Onychauxis).

In a metabolic disorder, the supply of important building materials to the nails is prevented. Fungal nail infections (onychomycosis), skin and thyroid disorders, various deficiencies or infections can also be causes.

In Onychogryposis (also known as onychogryphosis or ram’s horn nails)(bent nails; claw nails) the nail plate no longer adheres to the nail bed. The nail is then a dirty yellow colour, but remains transparent. This is probably triggered by hyperkeratosis (the thickening of the stratum corneum) (the outermost layer of the epidermis), of the nail bed which could be caused by circulatory disorders in the end flow zones.

If the foot specialist finds a yellowish discolouration, the medical history of the client is taken.
